Unlock by beating The Catacombs on Hard, then enter from hub. Run Nightmare (level 25+ reported) to hunt Awakened Devil / Devil Heart drops.
Overview
Also called Frost Spire / Frostspire Bastion. Mid-to-late dungeon unlocked after Catacombs Hard. Nightmare hosts Awakened Devil for Devil Hearts used by ADE and Jetstream quests.
